MIDDLETOWN, NY (October 10, 2019) – McKnight’s Long-Term Care News announced recently that Nexus Health Resources client Genesis HealthCare was among a select group of winners who earned a 2019 McKnight’s Excellence in Technology Award. Genesis received the Bronze Award in the Transitions category for its use of NexusConnexions transitional care software to improve transition-to-home outcomes.

In December, Genesis partnered with Nexus Health Resources to enhance its existing Genesis Care Transitions program. The provider implemented the NexusConnexions platform in 18 of its high-volume centers to standardize patient follow-up activities and to identify and track the resolution of patient issues in real time.

Hauswald, Senior Vice President of Strategic Development for Genesis HealthCare, and Ryan Sparks, Chief Revenue Officer for Nexus Health Resources, recently teamed up to present a webinar titled, Transition-to-Home Best Practices for Reducing SNF Readmission Penalties and Strengthening Patient Relationships. The one-hour presentation, which aired September 24th, was presented in conjunction with McKnight’s Long-Term Care News.

Hauswald and Sparks discussed how skilled nursing facilities (SNFs) are increasingly impacted by patient outcomes that occur after patients have left their building and presented best practices for developing a successful transition-to-home program. These include the need for post-discharge outreach and workflows to reduce the Skilled Nursing Facility Readmission Measure (SNFRM) penalty and strategies to strengthen relationships with patients and their families.

When asked during the Q & A segment what the biggest takeaway was for skilled care operators, Hauswald had this advice: “The one thing I would want to make sure folks do is do something. Whether it’s a simple follow-up call program or something more robust, it’s imperative that providers really accept this challenge of managing complex, chronic conditions and patients.”
