The Nexus Health Call Center makes live calls to patients after discharge to ensure the best possible outcomes. Customizable call agendas are configured with built-in escalation paths that provide real-time updates on changes in patient condition so that key members of HCR ManorCare staff are alerted when further intervention is necessary. Calls are made on weekends and evenings to ensure patient needs are addressed in a timely manner.
