Genesis HealthCare partnered with Nexus Health Resources to scale and streamline transitional care activities across a broader patient population and to identify and quickly address issues that could impede patient recovery at home.

MIDDLETOWN, NY (October 10, 2019) – McKnight’s Long-Term Care News announced recently that Nexus Health Resources client Genesis HealthCare was among a select group of winners who earned a 2019 McKnight’s Excellence in Technology Award. Genesis received the Bronze Award in the Transitions category for its use of NexusConnexions transitional care software to improve transition-to-home outcomes.

In December, Genesis partnered with Nexus Health Resources to enhance its existing Genesis Care Transitions program. The provider implemented the NexusConnexions platform in 18 of its high-volume centers to standardize patient follow-up activities and to identify and track the resolution of patient issues in real time.

“NexusConnexions allowed us to expand upon our existing Genesis Care Transitions program and focus on a larger patient population,” said Barbara J. Hauswald, Senior Vice President of Strategic Development for Genesis HealthCare. “As a result, we were able to more effectively track patient progress after they left our care and intervene if necessary to prevent readmissions.”

Using the NexusConnexions platform in conjunction with the Nexus Health Call Center, the Genesis Care Transitions team was able to confirm that over 70% of patients or their caregivers were reached via live telephone calls after returning home. This resulted in improved patient outcomes, fewer unnecessary hospital readmissions, and increased referrals to a Genesis facility. The facilities using the NexusConnexions software also saw a significant increase in patient satisfaction and positive social media reviews.

The McKnight’s Excellence in Technology Awards Program recognizes organizations harnessing technology to make eldercare better. In 2018 the program was expanded to include Skilled Care and Senior Living.
