The Redstone Highlands continuing care retirement community will use NexusConnexions® transitional care software to positively influence care plan adherence and promote the continued recovery of patients after they have been discharged.

MIDDLETOWN, NY (August 29, 2019) – Greensburg, Pennsylvania-based Redstone Presbyterian SeniorCare is a leading provider of premier living solutions and high-quality services for older adults. The non-profit organization has partnered with Nexus Health Resources to begin using its NexusConnexions® transitional care software at the Redstone Highlands continuing care retirement community.

Redstone offers distinct programs, including independent living, personal care services and in-patient rehabilitation on their Greensburg campus, Home and Community based services, through Redstone@Home Home Health and Hospice. They will implement the NexusConnexions software solution to more efficiently track and manage post-discharge follow-up activities and ensure quality health outcomes for patients who transition out of the Rehab Center at Redstone.

Patient and caregiver interactions are tracked using NexusConnexions reports to compare actual and target goals and see what steps resulted in preventing readmissions.
